Although those figures may seem shocking, most experts agree that the real number of women whose first sexual experience is rape is likely much higher. “That number is actually only the tip of the iceberg, because only women ages 18 to 44 were interviewed; if the interviews had included the whole population of women, that number would be millions higher,” says Laura Hawks, MD, lead author of the study and research fellow at the Cambridge Health Alliance in Massachusetts. The research, published on Monday, September 16, 2019, in JAMA Internal Medicine, used information collected in the annual survey conducted by the Centers for Disease Control and Prevention (CDC) known as the National Survey of Family Growth to explore the prevalence of forced sexual initiation among women and girls in the United States and whether this assault was associated with reproductive, gynecologic, and general health outcomes.

The majority of the survey is about family structure, desire to have children, and childhood experiences, as well as financial issues related to healthcare, says Dr. Hawks.

“The questions include things like, ‘How much money do you spend on healthcare?’ and ‘Have you ever not seen a doctor when you needed to because of cost?’” she says. When these questions are validated using other sources, they’re shown to be quite accurate, adds Hawks. The more personal questions about sexual history — including questions about forced or involuntary first-time sex — come at the end of the interview, says Hawks.

“These more sensitive questions are answered in a computer interview format so that the person can have more of an anonymous interviewing experience; this technology has been shown to increase honesty in answering these types of questions,” says Hawks. Some Differences Between Voluntary and Forced Sexual Initiation
Researchers used the information obtained in the survey to compare women whose first sexual experience was voluntary with women who had a forced sexual initiation. The average age of women who had forced first-time sex was 15 compared with age 17 for women whose sexual initiation was voluntary. The mean age of the men who forced sex was 27; the mean age for men who engaged in a consensual sexual initiation was 21.For women who were forced into sex, 26 percent were physically threatened during the encounter, and 46 percent reported that they were physically held down.Fifty-six percent of women were verbally pressured into having sex.

Lawson, an associate professor of obstetrics and gynecology and psychiatry and behavioral sciences at Feinberg School of Medicine at Northwestern University in Chicago, was not involved in this research. Why the Actual Numbers of Forced Sex Are Likely Higher
Two factors that lead to lower estimates are the underreporting of sexual assault formally to police departments as well as the fact that lot of women really don’t want to see themselves as victims, says Lawson.

“Most of us in the field who study violence against women and men would see this number and immediately think, ‘Wow, if that’s how many people are reporting this, there must be so many more,’” she says. This manuscript gives us a very helpful starting point, adds Lawson. Many women, even young women, might reframe their first experiences in a way that helps them view the sexual experience as not forced if it didn’t involve threats of violence or threats of any kind, says Lawson.

“It’s not difficult to imagine that many women might reimagine what happened to them in order to have the encounter be unpleasant yet consensual to avoid seeing themselves as victims,” she says. Another factor that suggests this estimate might be low is that the data was collected before the #MeToo movement, says Hawks. “While public opinion may be changing about how women identify their own sexual experiences, that public discourse would actually not have influenced our survey results [since it hadn't happened yet],” says Hawks.

Rape and Sexual Violence All About Power and Vulnerability
It’s very concerning that the average age for this forced encounter is 15, as is the discrepancy in age between the women and her assailant, whose average age was 27, says Hawks. “Women are being forced into sex by men who are certainly much older and presumably somewhat bigger than they are,” she says. Sexual violence happens to women of all races and all socioeconomic demographics and is unacceptably high in all categories, says Hawks.

“That said, what we’re talking about here is rape and sexual violence, which is all about power and vulnerability,” she says. Often, women of color and women of lower socioeconomic status are more vulnerable, and there were higher proportions of forced sexual initiation among those populations, says Hawks.

Rape Associated With Health Problems Later in Life
Researchers found a significantly high association between forced sex the first time and a number of health outcomes later on in life. Unwanted pregnancies, abortions, pelvic inflammatory disease, and poor health were all significantly associated with forced sexual initiation, says Hawks.

Although it’s interesting to see the relationship between these variables, it’s difficult to know from this study if these health conditions were caused or worsened by forced sexual initiation, says Lawson. “We don’t know if individuals from this study who experienced forced sexual initiation had differences in their demographic characteristics that could contribute to some of the correlations when compared with women who had nonforced sexual initiation,” says Lawson.

“Is that group more likely to have an abortion because they had experienced forced sexual initiation, or could they be part of a lower socioeconomic group who couldn’t afford birth control or had different education regarding prevention of unwanted pregnancies?” she asks. “Certainly, we know that sexual assault at any time, whether it is the first time that someone is engaging in sexual activity or some later time, can lead to a host of psychological conditions as well as some physical conditions,” says Lawson.

Education and Communication Necessary Steps to Reduce Rape
This is an issue that needs to be urgently addressed, because every week, thousands of women are experiencing rape at sexual initiation, says Hawks. “When we talk about sexual violence, we’re talking about a power imbalance between women and men, so from a societal perspective we really need to address how we treat women and men in every single sphere,” she says. This starts in grade school, says Hawks.

“We really need to be intentional to make sure we’re treating little girls the same way we’re treating little boys and continue to do that throughout their education and into the workplace,” she says. Another commonsense measure is to educate adolescents about sexual consent, says Hawks.

“We need to give both men and women the tools to have this kind of conversation,” she says. In medicine, this information can be used to improve care, says Hawks. “It’s important and vital for clinicians to understand just how common sexual violence is and how far-reaching the consequences of sexual violence can be,” she says.
