Can a Sitz Bath Help Ease Endometriosis Pain

What Is a Sitz Bath
During a regular bath, you immerse your entire body in water, which has the benefit of soothing all your muscles; a sitz bath, in comparison, offers more targeted therapy — only part of your body is soaked. Some women may be familiar with sitz baths if they were instructed to use them after childbirth. For this type of sitz bath, you use a shallow plastic bowl with a rim and an overflow hole, a plastic bag, and attached tubing.

The bag is filled with water, and the bowl is placed on the toilet seat. When you sit on the seat and elevate the bag (typically by clipping it to a towel rack or a nearby hook), the water flows onto your perineum to provide pain relief. While this type of sitz bath may also feel good to someone with endometriosis pain, naturopaths — doctors who emphasize herbs, nutrition, and other natural methods that encourage the body to heal itself — refer to these as a perineal bath.

Amanika Kumar, MD, a gynecologic oncologist at the Mayo Clinic in Rochester, Minnesota, says that some of her patients use sitz baths and that the baths can help reduce aches and be relaxing for a woman who is in pain. “In my practice, as well as from teaching hydrotherapy [treatment with water] for the past 10 years, I am more convinced than ever that it works,” says Kristina Conner, ND, a naturopath in Oakbrook Terrace, Illinois, and a professor of naturopathy at the National University of Health Sciences in Lombard, Illinois.

Although research hasn’t proven its effectiveness in healing these conditions, a review published in 2011 in the Japan Journal of Nursing Science found that patients said they felt satisfied with the way sitz baths helped their symptoms. Water Temperature Makes a Difference
Naturopaths recommend varying water temperatures for sitz baths, depending on the condition being treated. Painful menstrual cramps and pelvic inflammatory disease, for example, are typically helped with a hot-water sitz bath.

Pregnant women, however, should avoid sitting in hot water, as it may raise their body temperature and pose potential health risks to the developing fetus. For endometriosis, neutral temperatures of 92 to 98 degrees F (slightly cooler than lukewarm) are generally recommended, and women are instructed to fill the tub to the level of their belly button. Because the neutral temperature is more pleasant than hot or cold water, Conner suggests relaxing in it as long as possible, anywhere from 15 minutes to two hours.

An Addition to Standard Treatment
A sitz bath is not a replacement for medications or other treatments your doctor may prescribe for your endometriosis. Oral hormone therapy or nonsteroidal anti-inflammatory medications (NSAIDs) have been proven to be effective in lessening endometriosis pain and reducing future scarring of endometrial tissue.
